Nuvia Enrollment Terms & Conditions
1. Eligibility
• Programs are designed for girls and young women ages **13–25**.
• Enrollment requires truthful completion of the form by the participant or a parent/guardian (if under 18).
2. Participation
Nuvia programs focus on **healing, education, and empowerment**. Participants are expected to:
• Attend sessions regularly. • Respect facilitators, staff, and peers. • Maintain confidentiality within healing circles.
3. Safety & Conduct
• Nuvia maintains a **zero-tolerance policy** toward harassment, violence, or discrimination. • Physical and emotional safety is our priority. Any behavior that compromises safety may result in removal from the program.
4. Confidentiality
• Information shared in enrollment forms and sessions is kept **strictly confidential**, except: • If required by law, or • If there is a risk of harm to self or others.
5. Parental/Guardian Consent (if under 18)
For participants under 18, a parent/guardian must consent to:
• Enrollment. • Participation in workshops and activities. • Use of safe transport services, if requested.
6. Transportation Transportation services are provided for accessibility and safety. Participants/guardians must agree to:
• Provide accurate pickup/drop-off information. • Follow Nuvia’s **transport policy** (e.g., be on time, follow driver instructions).
7. Data & Media
• Nuvia may collect anonymized data for monitoring & evaluation. • Media (photos/videos) will only be used with **separate written consent** from participants or guardians.
8. Voluntary Participation
• Enrollment is voluntary, and participants may withdraw at any time by notifying Nuvia staff.
9. Agreement
